South Carolina’s Community Long Term Care (CLTC) program provides services to help eligible adults remain at home or in the community instead of moving to a nursing facility. Depending on your assessment and approval, CLTC may help cover certain in-home services that support daily living and safety.
If you’re a family member or case manager supporting a CLTC client, we’re here to help with the homecare side.
Actual approved services depend on the CLTC assessment and care plan, but in many cases, homecare under CLTC may include:

Help with bathing, dressing, and grooming

Assistance with meals and light housekeeping

Supervision and safety support

Help with mobility and transfers

Limited skilled nursing services in some situations, based on the care plan
Exact services and hours are determined by CLTC, not by USHealthcareNurses. We help you find providers who can deliver the services authorized in your plan.
